## Short answer

A 2025 meta-analysis of 31 comparative studies (1,426 patients) found that adding adipose-derived stem cells to fat grafts improved fat retention by 26.8 percentage points on average. For stromal vascular fraction (SVF) methods, the type usually sold as "stem cell breast augmentation", the gain was 17.0 points, rated low-certainty evidence. Complication rates did not differ, and the pooled population was mostly breast reconstruction patients.

## Answer

Culture-expanded stem cells produced the largest gain (64.6 points), while SVF isolated at the time of surgery produced a moderate one (17.0 points, I²=78%)[1]. Culture-expanded cells are regulated as a separate class of therapy in many countries, so it matters which of the two a clinic is actually offering[1].

Complications were 18.4% with stem cell enhancement versus 17.2% without (risk ratio 1.07). Among 813 patients with previous breast cancer, recurrence did not differ significantly, but the confidence interval was very wide and the authors rated oncologic evidence low-certainty[1]. The benefit tended to diminish after 18 to 24 months[1].

For comparison, a separate meta-analysis of 47 studies of conventional fat grafting to the native breast reported average graft retention of about 53% and fat necrosis in about 4.7% of patients[2].
